FQHC wrap-around reconciliation, tracked and recovered.
When Medi-Cal managed care pays less than your per-visit PPS rate, the difference is supposed to come back to you. Recovering it takes encounter-level tracking that general billers simply don't do.
What is FQHC wrap-around reconciliation?
Wrap-around reconciliation is the process of recovering the difference between what a Medi-Cal managed-care plan pays your health center and the per-visit PPS rate you're entitled to. The state reconciles that gap, but only for encounters that were correctly counted and claimed. Encounters that are missed are simply money your health center never receives.
Two payments, one visit.
Understanding this is what separates a health-center biller from a general one, because everything downstream depends on it.
You're paid per visit, not per procedure
Under the Prospective Payment System a health center earns a fixed per-visit rate for a qualifying encounter. Coding the encounter correctly is what protects that rate.
Managed care usually pays less
The plan pays its own negotiated or capitated amount, which typically sits below your PPS rate. That's expected, and it isn't the end of the transaction.
The difference is the wrap
The supplemental payment closes the gap between what the plan paid and your full rate. It is money you already earned by seeing the patient.
But only for encounters that were counted
The reconciliation can only return money for visits that were captured, coded, and claimed correctly. An uncounted encounter is invisible to the process.
Primary sources: the Medicare FQHC Prospective Payment System (CMS) and Medi-Cal FQHC reimbursement and reconciliation policy (California DHCS). Filing windows and reconciliation requirements are set by the state and change over time, so verify current deadlines with DHCS.
How we reconcile it.
Wrap-around isn't a year-end project. It is a continuous match between what you did, what you were paid, and what you were owed.
Capture every encounter
Each qualifying visit is counted and coded properly, because an uncounted encounter usually can't be reconciled once the deadlines pass.
Match payments to encounters
We tie each managed-care payment back to the specific encounter it belongs to, rather than treating payments as a lump sum.
Measure the gap
Payment against your PPS rate, encounter by encounter, so the shortfall is a number rather than a suspicion.
Claim and reconcile
Supplemental claims are filed and the periodic reconciliation is prepared from clean data, not rebuilt from memory.
How the money goes missing.
None of these set off an alarm. That's exactly why the loss can run for years before anyone notices.
Encounters never counted
A visit that was not captured as a qualifying encounter can't generate a wrap payment. The revenue is invisible from that point on.
Payments never matched
When plan payments are posted in bulk and not tied back to encounters, there's no way to know which visits were underpaid.
Supplemental claims not filed
The gap is identified but nobody files for it, so the money simply stays with the state or the plan.
Reconciliation done from scratch
If the data was not kept clean all year, reconciliation becomes an archaeology project, and whatever can't be proven isn't paid.
Filing windows missed
Reconciliation and supplemental claims have deadlines. Once a window closes, the shortfall becomes permanent.
Rate changes not applied
When your rate changes and billing keeps using the old figure, every encounter after that is quietly reconciled against the wrong number.
Health centers keep us for years.
Wrap-around is exactly the kind of work that compounds. The longer a specialist runs it, the cleaner your data and your reconciliation get.

Wrap-around questions.
Encounters that are missed are money you never receive.
Why do managed-care plans pay less than our PPS rate?
Managed-care plans pay their own negotiated or capitated amount, which is usually below your health center per-visit PPS rate. The difference isn't written off. It is supposed to come back to you as a supplemental or wrap payment, which is exactly the piece generic billers tend to leave behind.
How does money go missing in wrap-around?
Usually quietly. Encounters aren't counted correctly, managed-care payments are never matched back to the encounters they belong to, supplemental claims aren't filed, or reconciliation isn't prepared properly. None of these produce an alarm, which is why the loss can run for years unnoticed.
Do you handle the annual reconciliation as well as ongoing claims?
Yes. Ongoing wrap tracking and the periodic reconciliation are the same job at different time scales. We keep encounter and payment data clean throughout the year so reconciliation is a straightforward exercise rather than a scramble to rebuild history.
What if we're on California APM instead of traditional PPS?
We bill and reconcile under both. California Alternative Payment Methodology, rolling out in cohorts since 2024, pays participating health centers a per-member-per-month rate instead of per visit, but encounters still have to be captured and reported accurately. We handle either model. See our FQHC billing page.
How would we know if we're losing wrap money today?
Start with a free revenue checkup. We look at whether your encounters, managed-care payments, and supplemental claims line up. If there's a gap, it shows up quickly, and you keep what we find whether or not you work with us.
