FQHC Billing · Denial Management

FQHC denial management that resolves, not just resubmits.

Most billing companies push a denied claim back through and hope. We find out why it denied, fix the cause, recover the money, and stop the same claim from denying next month.

What is denial management in FQHC billing?

Denial management is the process of finding out why claims were rejected, fixing the underlying cause, recovering the money, and changing the process so the same denial stops happening. For health centers it matters more than for most practices, because Medi-Cal encounter and PPS rules create denial patterns that a general medical biller will not recognize.

The difference

Resubmitting isn't working a denial.

This is the single biggest gap between a specialist and a general biller, and it is measurable in your revenue.

Resubmitting sends the same claim back

Nothing about the claim changed, so the plan usually reaches the same conclusion. The work gets repeated and the revenue still doesn't arrive.

Resolving asks why it denied

Eligibility, plan routing, documentation, a visit rule, a coding choice. The reason determines the fix, and the fix determines whether it happens again.

Flat denial volume is the tell

If your denial numbers look the same every month, claims are being recycled rather than fixed. Real denial management shows up as a falling repeat rate.

And the clock is running

Every denial has a filing and appeal window. Claims that get recycled instead of resolved eventually age out, and then the money is gone for good.

Our method

How we work a denial.

The same five steps every time, so nothing depends on who happens to pick up the claim.

01

Identify quickly

Denials and rejections are caught early rather than discovered weeks later when the filing window is already closing.

02

Categorize by real reason

Not just "denied." We group by the actual cause, because that is the only way a pattern becomes visible.

03

Find the root cause

Was it front-desk eligibility, a plan-specific rule, documentation, or coding? The answer decides the correct fix.

04

Correct, appeal, recover

We rebuild the claim properly and appeal where appropriate, so the earned revenue actually comes in.

Step 05, the one that matters most: we fix the process at the source, so the same denial stops arriving.

What we see most

The denials health centers actually get.

These are the recurring ones in Medi-Cal safety-net billing, and what usually sits behind them.

Eligibility and wrong plan

The patient was not active on the date of service, or the claim went to the wrong managed-care plan. Verification before submission catches most of these before they go out.

Registration and demographics

A mistyped ID, name, or date of birth. Small front-desk errors that reliably produce denials downstream.

Authorization or referral missing

The service required an authorization that was never obtained or never attached to the claim.

Visit and encounter rules

Health-center specific rules, such as how a second same-day visit is treated, that general billers routinely get wrong.

Coding and documentation gaps

The code doesn't match what the documentation supports, or the record doesn't carry the detail the payer expects.

Coordination of benefits

Dual-eligible patients billed in the wrong order, so the claim denies until the sequence is corrected.

Duplicate claims

Often a symptom of resubmitting rather than resolving, which can compound the original problem.

Timely filing

The hardest one to fix, because once the window closes the revenue is gone. Aged claims have to be worked before that point.

Underpayment, not denial

Some claims pay, just not correctly. Managed care paying under your PPS rate is a gap that has to be reconciled. See wrap-around reconciliation.

Measured, not guessed

What we watch, and what you see.

We track denial rate, the top denial reasons, first-pass acceptance, what has been appealed, and what has been recovered. All of it is visible to your leadership in Medi-Net in real time, plus clear monthly reporting. See the platform.

Questions, answered

Denial management questions.

What is denial management in FQHC billing?

Denial management is the process of finding out why claims were rejected, fixing the underlying cause, recovering the money, and changing the process so the same denial stops happening. For health centers it matters more than for most practices, because Medi-Cal encounter and PPS rules create denial patterns a general biller will not recognize.

How is resolving a denial different from resubmitting one?

Resubmitting sends the same claim back and hopes for a different answer. Resolving asks why it denied, corrects the cause, and prevents the next one. If your denial volume stays flat month after month, that is usually a sign claims are being resubmitted rather than resolved.

What are the most common FQHC claim denials?

The recurring ones are eligibility and wrong-plan errors, registration and demographic mistakes, missing authorization or referral, duplicate claims, encounter and visit-rule problems such as a second same-day visit, coding and documentation gaps, coordination of benefits on dual-eligible patients, and timely filing.

Do you work our old denied claims when we switch to you?

Yes. Working your existing aged and denied claims is part of the transition, not an add-on. Money that is sitting in old denials is often the fastest recovery available, as long as it is worked before the filing window closes.

How do you keep denials from coming back?

We track denials by reason, not just by count, so the pattern is visible. When one reason keeps repeating we fix it at the source, whether that is front-desk eligibility capture, documentation, coding, or how a specific plan wants the claim built.

Will we be able to see our denial numbers?

Yes. Your denial rate, top denial reasons, and what is being worked are visible in our Medi-Net platform in real time, plus clear monthly reporting for leadership. You should be able to see what happened to any claim.
